Checklist

All students listed are students to whom I personally give regular individual instruction.
I understand that if my students enter the competition, I am required to fulfill judging responsibilities.
I have read the audition rules and understand the repertoire requirements.
I have had each student verify the information on the entry form and understand that if the information submitted is found to be incorrect, the student will be disqualified. Student’s signature is not required.
I have checked with each accompanist to verify that they are not playing for more than a total of TEN STUDENTS. The accompanist may play for either High School and Adult students or for College and Graduate students. (Please note the change from last year).
I have collected all student fees and have submitted payment to paypal on the GHNATS website for the total amount.

Photos from our events may be posted on the chapter webpage. Please let a chapter officer know if you do not want to be part of group photos.

DEADLINE – Entry form must be completed online by Monday, March 19, 2012.
